 Authorization Information for Radiological Services
The prior authorization program for outpatient diagnostic imaging services became effective March 1, 2007. All providers should have received the provider letter and reference guide in early February 2007.

to find updated information on a member’s authorization information — including
date called, date approved, exam category, valid billing codes (CPT) and more. To
